1.    Individual and team event fees submitted via check to the Ken Caryl Swim Club.
2.    $35 dues submitted via check to Colorado State Summer Club Swimming Association  (CSSCSA)
3.    The Eligibility Statement, Coaches Liability Insurance Statement and Sportsmanship Statement (Click Here).
4.    Sportsmanship Agreement Statement (Click Here).
5.    A hard copy of all entries.
6.    Proof of time for all swimmers, copies of meet results. 

No swimmer will be permitted to swim until team event fees and CSSCSA dues are paid in full. NO EXCEPTIONS!
